WINFIELD

*Provide free basic eye exm services to persons who meet the eligibility requirements. Eye glasses may be provided at no cost or a small fee in some locations. No contacts. 

*To be eligible the individual must have NO insurance (including Medicaid and Medicare), have income below the guidelines listed online, have not had an eye exam in the past 24 months, be a legal resident, and have not received a doctor referral through the Vision USA program in the last two years.

*Applicants must work with a charitable organization, social worker, case worker, or community health agency. All applicants must be submitted online by a referral agency. 

*Will serve a maxium of four members per household per year
